My tax disc has expired,what documents do I need if I'm taking to a dvla office?

My tax has expired and due to moving house have misplaced my logbook and tax reminder form so therefore can not renew car tax online or in post office so I'm going to head to dvla office tomorrow,does anyone know if I can get away with just having my insurance/mot? And would it have to be me or could my partner do it? Thanks in advance
