Why Doctors Dislike Insurance Co?

In America, why do doctors dislike government & private insurance companies? They always say that their underpaid and complain about money. But they earn over 6 figures in private practice, and salaried docs. Even in GP or a specialty they still complain. Arent doctors rich millions that make a lot of money on insurance from insurance companies?.
